Academic Suspension Appeal Form – Undergraduate Students
Northwestern State University policy allows a student with mitigating circumstances to appeal suspension from the University.
- Please complete page one of the form in its entirety. Page two will be completed by the college dean.
- In a TYPED letter, explain the basis of the appeal as well as a plan of action to improve your academic standing.
- Schedule a meeting with your academic advisor or department head.

- In a TYPED letter, explain why you did not make progress and a plan of action on how you will improve your academic standing.
- Provide any and all documentation for circumstances outside of your control.
- If graduating, provide a statement from your academic advisor.
